If the initial course of therapy was initiated elsewhere and the patient transferred to the reporting practice for continued treatment during the initial course of therapy, select ‘Patient transferred to reporting practice during the initial course of medical oncology treatment’
If the initial course of treatment was completed prior to patient transferring to reporting practice, select ‘Patient transferred to reporting practice following completion of initial course of medical oncology treatment’
