Site

MEDONC Cases If applicable, select the site the patient received their first cycle of anti-cancer medical oncology therapy. If the patient did not receive their first cycle of chemotherapy at any of the practice sites (received initial treatment elsewhere, i.e., at another health organization), select the site the patient first received any anticancer therapy at this practice. Select the site where the patient was first seen if the patient did not receive any anticancer therapy at this practice.

Practice Site

Individual practices may have specific guidance regarding site collection when a practice has multiple sites. Often, the location the patient receives treatment would be the site abstracted. If a patient does not receive treatment, choose the location at which the patient was first seen by practice.

For GYN ONC cases, select the site where the patient had their debulking surgery. If the surgery site can not be chosen, select the location where the patient received their first anticancer treatment. If the patient did not receive any anticancer therapy at this practice, select the location where the patient was first seen by the practice.

For END of LIFE cases, choose the location where the patient was last seen by the practice.

Managing/Treating Physician

The physician primarily responsible for the management of the oncologic care.

If two MedONC physicians are involved then follow the below guidelines for selecting the MEDONC physician:

  • If this is an end of life patient, please enter the physician that cared for the patient at end of life.
  • If it is not an end of life patient, please enter the physician that prescribed the initial chemotherapy course.
  • If that physician cannot be chosen, please enter the physician seeing the patient at this practice.

Was there a second oncologist involved in the patient’s care?

THIS QUESTION APPLIES TO GYN ONC CASES ONLY - the gynecologic oncologic surgeon should be the first physician listed, the medical oncologist would be the second oncologist listed if a patient receives chemotherapy managed by the medical oncologist.

• If there are two distinct breast cancer primary diagnoses in the past year, this chart meets selection criteria for abstraction.
• Do not enter ICD-10 codes related to symptoms or toxicities.
• ICD-10 codes are only accepted if within the invasive malignancy range provided.
• Use the most relevant code for the purpose of the abstraction. For example, use the code for the patient’s specific type of cancer, even if the most recent recorded visit denotes some other condition
