If a patient is non adherent; Examples include: a note to indicate that a discussion occurred with the patient to promote future adherence, that an intervention occurred to address stated barriers (i.e. referral to a financial counselor if coverage was an issue, symptom support or dose adjustment if side effects were causing them to skip doses on their own, etc). If one of the indicators of non-adherence was documented but then not addressed at all in the plan, it not addressed.
