Hysterectomy

Hysterectomy will populate if HYSTERECTOMY was selected on F-Surgery 1 page. Indicate if patient had a Simple, Radical, or Supracervical hysterectomy.

Total (simple) Hysterectomy

This is the removal of the uterine fundus (body) and cervix. This may include the removal of the tubes and/or ovaries and is described as salpingectomy (tubes only), oophorectomy (ovaries only), or salpingo-oophorectomy (tubes and ovaries). May be identified as ‘Total Abdominal Hysterectomy’ (TAH), ‘Total Laparoscopic Hysterectomy (TLH) or ‘Laparoscopic Hysterectomy’. The operative report frequently will include a description of the procedure performed.

  • Example: Procedure(s) Performed: DaVinci assisted total abdominal hysterectomy.

Radical Hysterectomy

The removal of the uterine fundus (body), cervix, the top portion of the vagina, and the parametria (tissue that surrounds the cervix). It may also include the removal of the pelvic lymph nodes. Sometimes, the description under the procedure performed requires further deliberation.

  • Example: DaVinci Assisted Total Abdominal hysterectomy, bilateral salpingo-oophorectomy, bilateral pelvic and para-aortic lymphadenectomy. Though not part of the description, this case qualifies as a radical hysterectomy.

Supracervical (subtotal) Hysterectomy

A supracervical hysterectomy, also known as a partial (subtotal) hysterectomy, is a surgical procedure where only the uterus is removed, leaving the cervix intact. This contrasts with a total hysterectomy, where both the uterus and cervix are removed.

Oophorectomy

Oophorectomy will populate if OOPHORECTOMY was selected on F-Surgery 1 page. Indicate if patient had a Left, Right or Bilateral Oophorectomy.

Lymph Node Dissection (select all that apply)

Lymph Node Dissection will populate if LYMPH NODE DISSECTION was selected on F-Surgery 1 page. Select "Lymph node sampling done” when Operative note mentions neither sentinel lymph node dissection, nor full lymph node dissection, but greater than or equal to 1 node has been removed. Please note that if you choose Not attempted, no other options should apply.

Vulvectomy

Vulvectomy will populate if VULVECTOMY was selected on F-Surgery 1 page. Indicate if the patient had a Right, Left, or Bilateral Simple or Radical Vulvectomy. If a vulvectomy is described as anterior or posterior, choose “bilateral”.

Bowel Surgery (select all that apply)

Bowel surgery will populate if OPEN surgical approach with BOWEL SURGERY was selected on F-Surgery 1 page. You may free type any procedure not listed in the "Other (Specify)".

Residual Disease

Indicate if there is documentation of residual disease in the operative note or after surgery, and if so, to what extent.

Residual Disease

Search terms for residual disease: No visible disease; residual; gross; tumor; disease; extrauterine; optimal debulking; suboptimal debulking; R0; R1; R2.  

~Suboptimal tumor debulking is considered <1 cm residual disease. Miliary disease is also considered <1 cm residual disease as miliary refers to “resembling or suggesting a small seed or many small seeds.”

~For endometrial cancers, gynecologic oncology surgeons do not typically report “no residual disease” as most cases only have cancer in the uterus (per MOQC Director of Gynecologic Oncology). Instead, documentation often reflects visual inspection of the abdominal cavity with no additional disease noted
